Chronic Conditions Management.

The management and prevention of chronic conditions is probably the single most important challenge to face policy makers and providers of health care today. There is, therefore, a need to make changes to service delivery and configuration. This framework for R&E proposes a multi-level and multi-strand approach.

As part of work to support research and evaluation of the Chronic Conditions Management (CCM) Policy in Wales, AWARD was commissioned by the Welsh Assembly Government to review the Local Health Board Chronic Conditions Action Plans prepared in May 2008. Welcome to AWARD

In June 2009, the All Wales Alliance for Research and Development in health and social care completed its five year programme of work. This website will remain available as an archive of AWARD activity and achievement, but will no longer be updated. Research teams in Swansea, Cardiff and Bangor Universities continue to carry out policy-relevant research on health and social care.

Our aim is to raise the profile of health and social care research in Wales by attracting research funding; collaborating with research networks, publishing findings; and developing research capacity in Wales.

AWARD is part of the Clinical Research Collaboration Cymru (CRC Cymru), funded by the Welsh Assembly Government through WORD. AWARD works closely with all parts of the collaboration; including the Thematic Research Networks and the Infrastructure Support Services.
